7. Copy of PSA Birth Contract (If Travelling with Immediate Family or If Travelling Alone)

1. Original Employment Certificate (Company Landline Number, Email Address, Company Address, Applicant’s Position, Applicant’s Compensation, Date Hired and Certificate Date Issued (3 months) 2. Copy if Recent ITR with BIR Stamp

IF BUSINESSMEN:
1. Copy of Business Registration from SEC or DTI (Include pages with name of stockholders)
2. Copy of Business Permit or Mayor’s Permit (must be updated)
3. Copy of Recent ITR or Form 2316 (Include pages with net income and total revenue)
IF STUDENTS:
1. Original School Certificate
2. Copy of School ID
3. Copy of Parent’s Documents (Refer to Employees and Businessmen)
4. Copy of Parent’s Passports
