4. Original PSA Birth Certificate (issued w/in 1 year)
5. Original NSO Marriage Contract (if married, issued w/in 1 year)
6. Daily schedule in Japan
7. 2 Original Individual Authorization Letter (1 for filing & 1 for releasing)
For Employed / Businessmen:
1.Bank Certificate (original)
2.Income Tax Return (photocopy)
3.Certificate of Employment (original) / Business Documents (photocopy)
For students:
1.School Id (photocopy)
2.Guarantee Letter (Japan Embassy format)
For Senior Citizen:
1.Senior Citizen Id (photocopy)
For Dependent/s:
1.Guarantee Letter (If a person other than the applicant will bear/shoulder the travel expenses, please submit a Guarantee Letter using the example/format provided by the embassy)
Additional Documents:
If Birth Certificate from PSA is unreadable, or If there is no record in PSA, together with “Certificate of Non-Record” from PSA Headquarter, - BIRTH CERTIFICATE FROM LOCAL CIVIL REGISTRAR (photocopy)
